Medical Release and Waiver of Liability
I give permission for my child to attend All Saints’ Episcopal Youth Community. In case of illness or accident, I give permission to have my child evaluated and treated by available medical personnel. I understand a reasonable attempt will be made to notify me in such an event. The adults in charge have permission to authorize any medical care, which in their judgment, they deem necessary and to sign any medical forms necessary on my child’s behalf, and I do hereby release the All Saints’ Episcopal Church and all persons connected with this group from any liability, claim, and expense related to any such condition, circumstance or treatment.
Transportation Release
I give permission to my child to ride in any vehicle designated by All Saints’ Episcopal Church employees and designated volunteers, while participating in and traveling to and from their events. And do hereby release, forever discharge, and agree to hold harmless All Saints’ Episcopal Church, its employees, volunteers, and agents from any and all liability, claims, or demands for accidental personal injury in the process of transportation.
Media Release
On behalf of my child the undersigned parent does agree to grant the All Saints’ Episcopal Church permission to record on film, video tape, or audio tape, the participation of the minor Child. The undersigned parent/guardian further agrees that any or all of the material recorded may be used, in any form, as part of any future productions made by the All Saints’ Episcopal church and further, that such use shall be without payment of fees, royalties, special credit, or other compensation to or for the benefit of Minor Child, parent, or any other person or entity.
